People We Serve
A Community residential facility (also known as an Adult Family Home or a Community Based Residential Facility) is a home in which unrelated older adults reside. Treatment or services above the level of room and board, including a minimal amount of skilled nursing care is provided. The target population of residents are:

  1. Advanced in age (> 60 years of age) 
  2. Irreversible dementia/ Alzheimer’s 
  3. Mild Mental Health Impairments 
  4. Ambulatory/ Semi ambulatory
  5. Developing Disabilities
  6. Traumatic Brain Injury

Caregivers face many challenges. Care giving is a demanding task, and it is easy to neglect your own health and well being when you are involved with your loved one’s needs. Caregiver’s need time off from their care giving responsibilities to relieve stress and prevent burnout.

Graceful Aging offers a safe, clean, home setting to bring your loved one to on an hour to hour or day to day basis. The resident can benefit from the friendship of the other residents and the functional assistance provided by our trained, professional caregivers.


Services for Residents

Community based support and services to people advanced in age suffering from irreversible dementia/ Alzheimer’s with or without mental illness. Graceful Aging offers a wide array of services, supports, and treatment options specifically designed for each resident’s individual needs. Specific services provided vary, but general services and supports include:

  • A clean home with fresh linens and toiletries 
  • Space in your bedroom to add your own personal touch 
  • Home cooked meals with dietary monitoring 
  • Onsite garden maintained by a gardener
  • Onsite laundry service 
  • Transportation to all doctor appointments
  • 24-hour a day supervision and support by CPR certified staff  
  • Transitional care 
  • Personal care assistance 
  • Physical care assistance 
  • Behavioral treatment and guidance 
  • Room and board 
  • Daily activities in the home and/or the community 
  • Health and medication management by a registered nurse 
  • Information and referrals in the community

  • Transporters trained to meet the needs of the physically and mentally disabled clients
