People in our program have choices, opportunities, professional care staff and a wonderful place to live in the community. The families, friends, and guardians of our residents are secure in knowing their loved ones are being provided with respectful, compassionate, professional, and safe environment.
Graceful Aging’s objective is to provide a home setting for the target group of residents that are not ready for nursing home placement, but do require supervision and minimum skilled nursing care. Using a team approach which includes the resident, family member, and/or guardian, facility manager and any other appropriate people as identified by the resident to develop an individualized service plan (ISP). The plan is written with participation from all members of the team and agreed upon by the team prior to implementation.
